We are glad that the Focus Pearl Program has been beneficial enough for you to consider applying for an extension. Some of these questions may seem repetitive to you; please understand that an extension may require that we reset goals, level of commitment, and attitudes. It is very important that you answer as honestly as possible.

Pearl Promise Programs & Services Qualifications

Incomplete applications will not be considered. Failure to complete the entire application will automatically disqualify you from receiving assistance. *Indicates a required field.

You must be a single parent with at least 50% legal custody of one or more children under 18 years of age
You must reside in Boulder County, preferably within the St. Vrain Valley School District.
You must currently be receiving some form of government or public assistance.
You must demonstrate motivation to become self-sufficient.

It will take approximately 60 minutes to complete this application.
Please provide as much detail as possible to help us understand your situation. Documentation (such as Proof of Residence, Employment, and/or copies of bills with account information) may be required.
Please note that submitting an application is the first step in the process and does not guarantee approval or fulfillment of the request. The selection process for the Focus Pearl program is highly competitive. Due to the number of applicants, limited spots available, and finite resources, we trust our clients to be willing and able to meet program requirements as described in the Focus Pearl Handbook.

I agree that my interactions with the Client Engagement Manager will be based on the principles of "A-C-T" - Accountability, Communication, Truthfulness.

I agree to monthly meetings with the Client Engagement Manager. If I am going to be late or miss an appointment. it is my responsibility to communicate and reschedule.

I recognize that this program is more than a monthly stipend and I agree to collaborate with the Client Engagement Manager to identify and implement a relevant projects that will both enhance my own skills and benefit the Pearl Promise organization. This may include working with newer Focus Pearls in a mentoring capacity, at the discretion of the CEM.

I agree that it is my responsibility to track progress on my own S.M.A.R.T. goals.

I agree to participate in Focus Pearl quarterly workshops aka "Culture Club" except under extenuating circumstances (approved by the CEM).

I agree to complete and submit a budget sheet that shows my income and expenses on a monthly basis.

I agree to inform the Client Engagement Manager of significant changes that occur that impact my income or my household needs. Examples: job loss, an increase or decrease in Housing, EBT, lack of food resources, car or health emergencies, etc.

I agree to volunteer for Pearl Promise events or activities as described in the Focus Pearl Volunteer Policy at a rate of one hour per month or three hours per quarter for a total of 12 hours in one year.
